Ipamorelin is a selective agonist at the ghrelin receptor, the same receptor the hunger hormone acts on. Its appeal in early research was selectivity: unlike older secretagogues, it stimulated growth hormone release without meaningfully raising cortisol or prolactin. That clean profile is the basis of most marketing copy written about it two decades later.

what the evidence shows

Very little in humans. Early-phase work explored it for post-operative ileus and it did not proceed. There are no published randomised trials establishing efficacy for body composition, recovery, sleep, or any of the outcomes it is now sold for. The pharmacology is well characterised; the clinical evidence is essentially absent.

Whether elevating growth hormone pulses in healthy adults produces any durable benefit at all, which is the unanswered question sitting underneath this entire category. In 2023 the FDA placed ipamorelin in Category 2 — bulk substances raising significant safety concerns — which bars licensed 503A compounding pharmacies from preparing it.

the practical note

Category 2 placement is the clearest signal the agency has published about these compounds, and it is the reason a legitimate compounding pharmacy will turn the request down while a research-chemical site will ship it the same day.
